Ⅰ. Guidelines for Passport Application

 For Permanent Residents:

1. Documents Required:

• Application for Passport : 1 original
  The Application may be downloaded.(HWP, MS-WORD, ACROBAT)
• Two(2) Color Photos
  - The photos on white background should be less than 6 months old. The size of the face is approximately 1¼" and should be the front view. The hats, head-gears, or sunglasses are not allowed.
  - Affix one photo on the Application for Passport and the remainder should be sent along with the application.
• A copy of the front and back of the Permanent Resident Card(if mailed, have the copy notarized).
• The current passport(original) and a copy of the current passport(the pages with personal information)
• Processing Fees: (in cash or money order, no personal checks)
  - Under age 8 : $20.-
  - 8 and older : $32.-
  - If mailed, include a pre-paid overnight envelope/airbill for the return of the passport.
  - You may make one money order for all the fees.

2  Following individuals have to Submit Additional Documents

    (1) Male Applicants between 18 and 35 years old:

• Proof of the completion or exemption from military services(a copy of the page of the passport on which the embarkation records is stamped or the Certificate of Completion)
• Applicants who have not completed military services have to apply for an extension of military service.

    (2) For the separation of accompanied children from the passport or if this application is the first application after reaching the age of 14:

• submit a copy of family register

    (3) Addition of Husband's Surname:

• submit copies of marriage certificate

3. Processing Time: by the category

• Applying in Person: approximately 7 days upon receipt of the application (About 2weeks in spring(March to June))
• First time applicant after reaching the age of 14: about 2weeks
• Applying by Mail: an extra delivery time is needed in addition to the above mentioned processing time.

For Non-Resident Aliens(Students, Company Reps, Short-Term Employees, and Others)

1. Documents Required:

• Application for Passport : 1 original
  The Application may be downloaded.(HWP, MS-WORD, ACROBAT)
• Two(2) Color Photos
  - The photos on white background should be less than 6 months old. The size of the face is approximately 1¼" and should be the front view. The hats, head-gears, or sunglasses are not allowed.
  - Affix one photo on the Application for Passport and the remainder should be sent along with the application.
• Additionally required documents:
  - Student: Certificate of Attendance(original) and I-20(copy)
  - Employee: Certificate of Employment
  - Others: INS's Authorization for stay
  - Undocumented aliens: grounds for the over stay & the pledge to try to expeditiously obtain the legal status.
• The current passport(original) and a copy of the current passport(the pages with personal information and visa section)
• Processing Fees: (in cash or money order, no personal checks)
  - Under age 8 : $30.-
  - 8 and older : $42.-
  - If mailed, include a pre-paid overnight envelope/airbill for the return of the passport.
  - You may make one money order for all the fees.

2. Following Individuals have to Submit Additional Documents

    (1) Male Applicant between 18 and 35 years old

• Proof of the completion or exemption from military services(a copy of the page of the passport on which the embarkation records is stamped or the Certificate of Completion
• Applicants who have not completed military services have to apply for an extension of military service.

   (2) For the separation of accompanied children from the passport or this application is the first application after reaching the age of 14

• submit a copy of Family Register

   (3) Accompanying family members

• submit a copy of Family Register or Resident Registration

• Applying in Person: approximately 7 days upon receipt of the application (About 2weeks in spring (March to June))

• Applying by Mail: an extra delivery time is needed in addition to the above mentioned processing time.

Additional Documents for Lost/ Stolen/ Damaged Passports

• Grounds for applying for the passport(MS-WORD, ACROBAT)
• Photo I.D.(Notarize it when copies are submitted)
• Wire fee : $10.-(in addition to the application fee)
  - in cash or money order, no personal checks
• Processing time : about 10days
• Application should be submitted at the Consulate in person.
  - One may submit the application by mail if the applicant resides in a long distance from the Consulate.  In this case the president of the area Korean Association can verify the identity of the applicant.
• If the term of validity of the lost/stolen/damaged passport is more than one year, the new passport will only carry the remaining validity of the old passport.  If the applicant prefers, a regular 5 years of validity can be given after referencing with the Korean Authorities.

Ⅱ. Extension of Validity, Addition of Accompanying

Family Members

- Request for extension of validity can be submitted 1 Year before/after the expiration date of the current passport.  However, the validity of passport can not exceed 10 years from the issue date.

- If the current passport is in an old format(*), submit a Passport Application instead of application for Extension, 2 photos, $5.-(cash or money order) all others are the same as the required documents needed for Extension Application.
* old format : the lime/pink colored inner pages
* new format : the peach/light violet colored inner pages

 For Permanent Residents:

 Ⅲ. Application for Single Passport

• Passport Application : 1original
  The Application may be downloaded.(HWP, MS-WORD, ACROBAT)
• Grounds for Travel Certificate
  The form can be downloaded.(MS-WORD, HWP)
• Two Color Photos
  - The photo on white background should be less than 6 months old. The size of the face is approximately 1¼" and should be the front view. The hats, head-gears, or sunglasses are not allowed.
• The current passport(original) and copies of the passport(the pages with personal information & Visa section)
• Processing Fees : $15.-(in cash or money order, no personal checks)
  - If mailed, include a pre-paid overnight envelope/airbill for the return of the passport.
  - You may make one money order for all the fees.
• Processing Time : Approximately 7 days upon receipt of the application(if applied by mail : delivery time should be added.)
• For Permanent Residents : a copy of the front and back of the Permanent Resident Card(if mailed, have the copy notarized).
• For persons returning to Korea permanently : a copy of airline ticket
• For Stolen/Lost Passport : Application for reissue of the passport(Consulate's form hwp format) and a Photo I.D., Wire Charge of $10.- and should appear in person at the Consulate
  - One may submit the application by mail if the applicant resides in a long distance from the Consulate.  In this case the president of the area Korean Association can verify the identity of the applicant.
  

 Ⅳ. Guidelines to fill out the forms

• Please sign on the Application.
• Korean Name : for married women - use maiden name
• Resident Registration Number : if not Known, write in the applicant's date of birth
• Head of Household : the name on the Family Register
• Permanent Address : for married women - use spouse's permanent address
• Last Address in Korea : address used for the Resident Registration
• Household Members : include all the household members residing either in Korea or in the U.S.(if married, include the names of spouse & children, if not married, include the names of parents & siblings)
• Accompanying children : only if they are included on the passport
